I acknowledge and accept that there is a risk that I could be exposed to COVID-19 while attending any events hosted or directed by Académie Extrava Danse Academy (EDA). I also acknowledge and accept that while receiving services, the dance instructor may need to be closer than the recommended social distancing guidelines in order to assess and/or correct my child. I acknowledge and confirm that I am willing to accept this risk as a condition of attending classes and receive services offered by Académie Extrava Danse Academy (EDA).

I agree to release Académie Extrava Danse Academy (EDA) and the Facility (if applicable), their instructors, directors, employees, agents, and volunteers (the “Releasees”) from any and all causes of action, claims, demands, requests, damages or any recourse whatsoever in respect of any personal injuries or other damages which may occur or arise as a result of exposure to COVID-19 during my visit to any of Académie Extrava Danse Academy dance classes, rehearsals, demonstrations, recitals, or events.

I do hereby acknowledge and agree that notwithstanding the generality of the foregoing, I declare that I will not commence litigation or otherwise seek to recover damages or other compensation against the Releasees based on any action, claim, demand, request, loss or any recourse whatsoever arising from any potential or actual exposure to COVID-19 while attending at the Facility and/or through the provision of services to me by Académie Extrava Danse Academy. I further acknowledge that the Releasees can rely on this Release of Liability, Waiver of all Possible Claims and Assumption of Risk as a complete defence to any and all claims, damages, causes of action, or recourse or liability that may arise at any time.

I have carefully reviewed this Release of Liability, Waiver of all Possible Claims and Assumption of Risk and acknowledge that I fully understand the terms as set out above. 

I acknowledge that I am signing this Release of Liability, Waiver of all Possible Claims and Assumption of Risk voluntarily. 

I also permit that material in which my child / I appear, such as photographs and recordings may be used in whole or in part by the media of television, internet, video, print or display for promotional, educational, or commercial purposes.

Assumption of Risk:

I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at Academie Extrava Danse Academy, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.

Release of Liability:

I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.

Studio Policies:

Please review and agree to our studio policies:

1. Parking lot- DO NOT EVER LET YOUR CHILD PLAY IN THE PARKING LOT. We often see parents chatting in the lobby after class while their child walks out the door without them. We also see parents chatting outside the studio while their child plays between cars or in the neighboring business parking. This is absolutely against studio policy. Children are not to be left outside without supervision or leave the building without an adult.
2. Where to park- We have designated parking spots; Please park in the designated parking spots leaving the entrance from the street open and not parking beside the building.
3. Send your student(s) to the studio prepared for class- Have the student(s) dressed in the necessary equipment
4. Before and after class- Most classes start as a class ends. Please gather your child and be prepared to leave the building after class is over. Please be courteous to the class that comes after your class is over.
5. Communication- The best way to communicate with teachers is by emailing info@extravadanse.ca . Phone messages may not be returned the same day. Teachers cannot accept calls during class time. Our website has information regarding studio events (look at all the sliders) and we post studio closures due to weather on the website.
